The insulin resistance blood test is a comprehensive panel that evaluates how effectively your body regulates blood sugar, helping to assess your risk of pre-diabetes and type 2 diabetes.

It includes three key measures: HbA1c, which reflects your average blood sugar level over the past 2–3 months; fasting glucose, which checks for impaired fasting glycaemia; and the HOMA-IR calculation, which indicates emerging or established insulin resistance.

The insulin resistance blood test is a comprehensive test that assesses how well your body can regulate blood sugar levels.

This helps to evaluate your  risk of pre-diabetes and type 2 diabetes. HbA1C gives an indication of your average blood sugar level over the last 2-3 months, fasting glucose tests for evidence of impaired fasting glycaemia, and the HOMA-IR calculation gives evidence of impending or established insulin resistance.

  1. CAN I SEE A GP TO DISCUSS MY RESULTS?

    Yes, usually the same or next day.


    One Day Tests has a fantastic clinical team of consultants and GPs. Once you have your results you can book an online video consultation to discuss your results.


    Our clinical team consists of practicing NHS and private GPs, who work closely with our laboratory and can tell you what your blood test results mean for you. Having an integrated laboratory and GP services makes your health journey seamless and extremely efficient.


    This service is not included in the price of the test, it is charged separately, should you wish to book it.

This is a fasting test. It should be taken in the morning having eaten nothing, and drunk only water, from 10pm the night before.

Clinic appointment - how to prepare for the Insulin Resistance (HOMA-IR) Blood Test

If you are coming into one of our clinics for the test, this is what you need to do before you arrive:

  • You need to fast for this test for 10 hours. Book it for the morning.
  • Drink plenty of water about an hour before the appointment.
  • Avoid coffee/tea/energy drinks. Water is good!
  • That’s it! We’ll do the rest.

Here are some of the most frequently asked questions about the Insulin Resistance (HOMA-IR) Blood Test.

Do I need to fast before a HOMA-IR test?

Yes, 8 to 12 hours, water only. HOMA-IR is calculated from fasting glucose and fasting insulin measured on the same sample, and the model assumes your body is in a basal, unstimulated state. Insulin has a plasma half-life of roughly 4 - 6 minutes and starts climbing within minutes of eating, so a snack, a coffee with milk, or a sugary drink can multiply the insulin figure several-fold and push the score into a misleading range.

Book a morning appointment, avoid alcohol for 24 hours and strenuous exercise on the day, and continue any prescribed medication unless your doctor says otherwise.

If you plan to retest, use the same time of day each time - diurnal variation in insulin is real and comparability matters more than a single reading. 

What is a normal HOMA-IR result?

Our laboratory (and other UK laboratories) report in SI units, so the calculation is:

HOMA-IR = (fasting insulin mIU/L × fasting glucose mmol/L) ÷ 22.5

As a general guide:

HOMA-IR Under 1.0: Optimal insulin sensitivity
HOMA-IR 1.0 – 1.9: Normal to early change
HOMA-IR 2.0 – 2.9: Probable insulin resistance
HOMA-IR 3.0 and above: Significant insulin resistance

There is no internationally agreed cut-off. Thresholds are population-specific and assay-specific, and several studies in South Asian populations use lower ones.

HOMA-IR is a surrogate marker validated against the euglycaemic clamp, it estimates insulin resistance for risk assessment and tracking. It does not diagnose diabetes; that's what HbA1c and the oral glucose tolerance test are for. Read it alongside HbA1c, triglyceride:HDL ratio (available in our Cholesterol Panel) and waist circumference rather than in isolation.
