TOTAL PSA BLOOD TEST OVERVIEW

Prostate Specific Antigen (PSA) is a protein that is produced by the prostate. Although the protein is produced by healthy prostate cells, levels can rise if there is a problem with the prostate, particularly in prostate cancer.

PSA is a useful screening test for prostate cancer but it is not perfect so you should be aware of its shortcomings. Many men will have a high PSA but not have prostate cancer. Testing the PSA in these men can result in them having invasive procedures, such as a prostate biopsy, when there is actually no problem.

Similarly, some men will have a "false negative" result, a normal PSA when there actually is underlying cancer.

For this reason, even if your PSA is normal, you should still see a doctor if you have symptoms such as difficulty passing urine, getting up lots at night to pee, or feel that you are not emptying your bladder properly.

TOTAL PSA BLOOD TEST

There are two biomarkers that can be measured in PSA - the total PSA level and the free PSA level. The total PSA test measures the total amount of PSA in the blood, including both bound and unbound forms. The free PSA test measures only the unbound, or free, PSA in the blood. It is important to have both of these biomarkers measured.

The ratio of these two markers can be a very good indicator for your GP as to what is happening and the likelihoods of current or future issues. This is calculated by dividing Free PSA by Total PSA and multiplying by 100, giving the Free to Total PSA ratio as a percentage.

    Whilst we accommodate the shortest of time scales, with some results available within hours, for this particular test the usual turnaround time is: 3 working days.

    Our clinical team consists of practicing NHS and private GPs, who work closely with our laboratory and can tell you what your blood test results mean for you. Having an integrated laboratory and GP services makes your health journey seamless and extremely efficient.

The total PSA blood test can be a valuable tool in detecting prostate cancer and other prostate-related conditions.

However, it is important to note that a high level of PSA in the blood does not necessarily mean that prostate cancer is involved.

Other factors, such as age, ethnicity, and certain medications, can also affect PSA levels and it is absolutely vital that you follow up your results with your GP.

Red flag symptoms for the Total PSA Blood Test

Even if your blood tests are normal, you should see a doctor if you have any of the symptoms below because they may be a sign of serious underlying illness. 

  • Frequent urination
  • Pain ejaculating
  • Blood in semen or urine
  • Progressively severe symptoms of any kind

SPECIAL INSTRUCTIONS

For the 48 hours before having a PSA test:

  • avoid sexual activity as ejaculation as it may raise PSA levels significantly (but temporarily).
  • avoid vigorous exercise in the previous 48 hours.
  • you do not have an active urinary infection (PSA may remain raised for many months)
  • you have not had a prostate biopsy in the previous 6 weeks

Clinic appointment - how to prepare for the Total PSA Blood Test

If you are coming into one of our clinics for the test, this is what you need to do before you arrive:

  • Read the instructions in the SPECIAL INSTRUCTIONS section
  • Drink plenty of water about an hour before the appointment.
  • Avoid coffee/tea/energy drinks. Water is good!
  • That’s it! We’ll do the rest.

Home test kit - how it works with the Total PSA Blood Test

Our blood test kits are simple and convenient.

The most important step in doing a blood test at home is to read the instructions very carefully. The process is very simple, but there are a few pitfalls you need to be aware of - you will save yourself a lot of time if you do it right the first time around!

  • Full printed instructions will arrive with your kit - please read them carefully.
  • Drink plenty before taking the sample and warm your hands up in the sink with warm water for a few minutes. This will make your blood flow much better.
  • Fill the tubes to the top line. We don’t need much blood; filling tubes to the top line is about 15 drops of blood.
  • Post the samples on the same day you take them, before midday, Monday to Thursday. If you post it any other time it might get to us late or be stuck at a sorting office over the weekend. You will then have to do it again.
  • Package the sample in the same box the kit came in. Otherwise, Royal Mail or DPD will reject it. The sample tubes go into a pouch, then into a cardboard box, and then into a return envelope. All of this is provided in the kit: please use it.
