BOOK SUBSCRIPTION CLINIC APPOINTMENT

If you have a subscription that requires a clinic visit for a blood draw, you can book it here.

PLEASE ENSURE YOUR APPOINTMENT DATE IS AFTER THE RENEWAL TIME FOR NEXT TEST. OTHERWISE THE PHLEBOTOMIST WILL NOT SEE YOUR NEW ORDER AT THE CLINIC, IF YOU ARRIVE BEFORE THE RENEWAL DATE AND YOU MAY BE CHARGED TWICE.

How to book your subscription appointment:
