Accutane (also known as Isotretinoin or Roaccutane ) is a widely recognised medication for treating severe acne. The treatment, however, requires thorough and ongoing monitoring. One of the most important steps before and during Accutane therapy is completing a comprehensive blood test to ensure the treatment remains both safe and effective. This blood work helps you and your doctor keep track of your body’s overall health, detect any early signs of unwanted side effects, and adjust dosages as necessary to protect your wellbeing, as well as remain effective.

A primary component of your Accutane blood test is the liver function test, which assesses how well your liver is filtering and processing substances in the body. Because Accutane can place additional strain on the liver, measuring liver enzymes like ALT and AST is critical. Elevated levels may indicate stress or damage, prompting your doctor to adjust the medication dosage or, in some cases, pause treatment to allow the liver to recover.

Another key element involves Urea & Electrolytes (U&Es) and a Full Blood Count (FBC). The U&Es panel measures kidney function and electrolyte balance by checking sodium, potassium, and creatinine levels. Meanwhile, the FBC tracks important blood cell counts, including red and white blood cells, as well as platelets. Monitoring these values helps ensure your kidneys continue to function properly and that your body has adequate blood cell levels to maintain a robust immune system during Accutane therapy.

Lastly, lipid testing evaluates cholesterol and triglyceride levels, which can sometimes rise when taking Accutane. Hi, I am Dr. Adam Staten and here's my view on this test as a GP and Clinical Director at One Day Tests.

Isotretinoin (also known by the brand names Accutane and Roaccutane) is a medication that is used to treat severe acne or acne that has failed to respond to other types of acne treatment.

In the UK, this medication is only prescribed by dermatologists. Although isotretinoin is very effective, it has some potential side effects that need to be monitored for throughout treatment. It is recommended that people undertake a blood test before they start isotretinoin, one month after starting, and then at three monthly intervals.

Our isotretinoin/roaccutane/accutane panel includes the relevant blood tests to ensure that your treatment can be safely continued.

Red flag symptoms for the Accutane Treatment Blood Test

Even if all your blood tests are normal, you should speak to a doctor if you have any of the symptoms below:

If you are coming into one of our clinics for the test, this is what you need to do before you arrive:

  • Drink plenty of water about an hour before the appointment.
  • Avoid coffee/tea/energy drinks. Water is good!
  • That’s it! We’ll do the rest.

Home test kit for Accutane Treatment Blood Test - how it works

Our blood test kits are simple and convenient.

The most important step in doing a blood test at home is to read the instructions very carefully. The process is very simple, but there are a few pitfalls you need to be aware of - you will save yourself a lot of time if you do it right the first time around!

  • Full printed instructions will arrive with your kit - please read them carefully.
  • Drink plenty before taking the sample and warm your hands up in the sink with warm water for a few minutes. This will make your blood flow much better.
  • Fill the tubes to the top line. We don’t need much blood; filling tubes to the top line is about 15 drops of blood.
  • Post the samples on the same day you take them, before midday, Monday to Thursday. If you post it any other time it might get to us late or be stuck at a sorting office over the weekend. You will then have to do it again.
  • Package the sample in the same box the kit came in. Otherwise, Royal Mail or DPD will reject it. The sample tubes go into a pouch, then into a cardboard box, and then into a return envelope. All of this is provided in the kit: please use it.

Why do your test Cholesterol?

It is important to keep an eye on your cholesterol levels as part of the accutane medication, which can be affected.

What is tested in this panel?

The first test in the panel is the Full Blood Count (FBC) because isotretinoin can cause blood abnormalities including anaemia, low white blood cells, and low platelets (small cells that are involved in blood clotting).

We include liver function tests as this medication can affect the liver and abnormalities in liver function may mean that treatment needs to be paused or stopped. Occasionally, the medication can affect the kidneys so our panel includes a kidney function test.

Lastly, isotretinoin can affect the levels of lipids in the blood. Rarely, if the level of these lipids gets very high, it can result in severe inflammation of the pancreas (pancreatitis), and therefore we check your blood lipid profile. 

Your results should be reviewed by your dermatologist at each consultation.
