Follicle Stimulating Hormone
FSH plays a central role in ovarian function. As the ovaries slow down, FSH levels rise. A higher FSH result suggests the ovaries are becoming less responsive. FSH is particularly useful for diagnosing premature menopause or early menopause in people under 45. However, in people aged 45 and over, FSH is less reliable due to natural fluctuations.
Oestrogen
Oestrogen levels fall as menopause approaches. An oestrogen test may help clarify whether symptoms are linked to declining hormone levels, especially when combined with FSH. It is not always used routinely, but it can offer additional insight when symptoms do not match the expected pattern.
Luteinising Hormone
LH also increases as ovarian function declines. While it is not used alone to diagnose menopause, it is often checked alongside FSH to provide a more complete hormonal picture.
Anti Mullerian Hormone
AMH reflects ovarian reserve. Lower levels suggest reduced ovarian function, which may be helpful information for those approaching menopause earlier than expected. AMH is not used to confirm menopause but can provide context for cycle changes.
What FSH and LH Levels Are Typical Before, During, and After Menopause
Hormone levels vary between individuals, but certain patterns are common during the menopause transition.
Premenopause
Before perimenopause begins, FSH and LH remain within their usual reproductive ranges. The ovaries respond predictably, oestrogen levels fluctuate in a regular pattern, and menstrual cycles tend to remain stable.
Perimenopause
During perimenopause, the ovaries begin to respond less consistently to hormonal signals.
This leads to fluctuations in oestrogen, irregular ovulation, and gradually rising FSH levels. FSH may occasionally rise above typical reproductive ranges, sometimes above 30 IU per litre. These increases are not constant, which is why interpreting a single progesterone, FSH, or LH test during perimenopause can be challenging.
LH may also rise but is often more variable. The hormonal picture can look different from month to month, even within the same person.
Postmenopause
After periods have stopped for 12 months, most people are considered postmenopausal. By this stage, ovarian hormone production has declined significantly. FSH levels are usually much higher than during the reproductive years, and LH levels are also elevated but tend to be lower than FSH.
Although ranges vary, it is common to see FSH levels well above 30 IU per litre in postmenopause. Some people have levels far higher, reflecting the long term decline of ovarian function. LH levels also rise but are generally less dramatically elevated.
These changes reflect the body settling into a new hormonal baseline.
Why Home Menopause Tests Are Not Accurate Indicators
FSH Fluctuates Significantly
FSH does not rise in a smooth or predictable way during perimenopause. It can be high one day and lower the next. A positive result may simply reflect a temporary rise rather than a true indication of menopause. A negative result may occur even when symptoms strongly suggest perimenopause.
Relying on a single FSH reading oversimplifies a dynamic and complex process.
FSH Alone Cannot Diagnose Menopause
Diagnosing menopause involves more than hormone levels. Age, symptoms, cycle changes, and medical history all play a central role. This is why many healthcare guidelines state that menopause is usually diagnosed clinically, not through testing, especially for people aged 45 and older.
