SEX HORMONE BINDING GLOBULIN (SHBG) BLOOD TEST

SHBG (Sex Hormone-Binding Globulin) is a protein produced mainly by the liver that binds to sex hormones in the bloodstream, primarily testosterone and oestrogen, and regulates their availability to body tissues.

Measuring SHBG levels helps assess how much free (active) hormone is available for use, which is especially important when total hormone levels appear normal but symptoms suggest an imbalance.

DOCTOR'S COMMENTS

Hi, I am Dr. Adam Staten and here's my view on this test as a GP and Clinical Director at One Day Tests.

DR. ADAM STATEN - GP VIEW AND RED FLAG SYMPTOMS

From a GP’s perspective, the SHBG blood test is a useful tool when your symptoms suggest a hormonal issue, but your standard hormone tests, like testosterone or oestrogen, don’t give the full picture.

SHBG acts like a carrier, binding to sex hormones and controlling how much is available for your body to use. If SHBG is too high or too low, it can affect hormone activity even when total hormone levels seem normal.

We often use this test when investigating problems such as low libido, irregular periods, fertility issues, or signs of hormonal imbalance like acne or excess hair growth.

It can also be helpful in cases involving thyroid issues, liver function problems, or metabolic concerns.

Red flag symptoms for the Sex Hormone Binding Globulin (SHBG) Blood Test

Even if your blood tests are normal, you should see a doctor if you have any of the symptoms below because they may be a sign of serious underlying illness.

  • Progressive muscle weakness
  • Unexplained weight loss
  • Progressively severe symptoms of any kind

If you are coming into one of our clinics for the test, this is what you need to do before you arrive:

  • Read the instructions in the SPECIAL INSTRUCTIONS section
  • Drink plenty of water about an hour before the appointment.
  • Avoid coffee/tea/energy drinks. Water is good!
  • That’s it! We’ll do the rest.

Home test kit - how it works with the Sex Hormone Binding Globulin (SHBG) Blood Test

Our blood test kits are simple and convenient.

The most important step in doing a blood test at home is to read the instructions very carefully. The process is very simple, but there are a few pitfalls you need to be aware of - you will save yourself a lot of time if you do it right the first time around!

  • Full printed instructions will arrive with your kit - please read them carefully.
  • Drink plenty before taking the sample and warm your hands up in the sink with warm water for a few minutes. This will make your blood flow much better.
  • Fill the tubes to the top line. We don’t need much blood; filling tubes to the top line is about 15 drops of blood.
  • Post the samples on the same day you take them, before midday, Monday to Thursday. If you post it any other time it might get to us late or be stuck at a sorting office over the weekend. You will then have to do it again.
  • Package the sample in the same box the kit came in. Otherwise, Royal Mail or DPD will reject it. The sample tubes go into a pouch, then into a cardboard box, and then into a return envelope. All of this is provided in the kit: please use it.

We look forward to receiving your sample in our lab!

FREQUENTLY ASKED QUESTIONS

Hi, I am Lana Nela, Lead Nurse, and here is what we are frequently asked about this test.

FREQUENTLY ASKED QUESTIONS

Here are some of the most frequently asked questions about the Sex Hormone Binding Globulin (SHBG) Blood Test.

What happens if my SHBG level is too high or too low?

Abnormal SHBG levels can affect how much active testosterone or oestrogen is available in the body.

High SHBG may lead to symptoms of hormone deficiency, while low SHBG may be linked to conditions like PCOS, obesity, or insulin resistance.

WHY HAVE THE TEST?

Hi, I am Nader Eldarf, I am a Biomedical Scientist in our laboratory and here is why it's important to take this test.
  1. WHY HAVE THE SEX HORMONE BINDING GLOBULIN (SHBG) BLOOD TEST?

The SHBG test is important because it helps uncover hidden hormonal imbalances by showing how much testosterone or oestrogen is actually available for use in the body.

SHBG binds to sex hormones in the blood, limiting the amount of "free" (active) hormone that can act on tissues.

Even if your total hormone levels appear normal, abnormal SHBG levels can mean you're experiencing either excessive or insufficient hormone activity, leading to symptoms like fatigue, low libido, irregular periods, or acne.

IS THIS TEST RIGHT FOR YOU?

Hi, I am Sandeep Patel, I am the Laboratory Director at One Day Tests. Here are the symptoms that you may wish to address with this test.
  1. IS THE SEX HORMONE BINDING GLOBULIN (SHBG) BLOOD TEST RIGHT FOR YOU?

The SHBG test may be right for you if you're experiencing symptoms that suggest a hormonal imbalance, but your total testosterone or oestrogen levels don't fully explain your condition. It’s particularly useful if you have:

In men - symptoms like low sex drive, fatigue, muscle loss, erectile dysfunction, or infertility

In women - irregular menstrual cycles, acne, excessive hair growth, or signs of PCOS.

In either sex - conditions like thyroid dysfunction, liver disease, or unexplained changes in energy, mood, or body composition.

The test helps determine how much of your sex hormones are biologically active by assessing how tightly they are bound to SHBG.

  1. SYMPTOMS AND TAGS

Chronic fatigue, Erectile disfunction, Fatigue, Hair loss, Lack of libido, Loss of muscle bulk and Low mood

WHY ARE BLOOD TESTS IMPORTANT?

Health monitoring is very important, you are in charge of your health - look after it.
