Our coeliac screen involves measuring several immunological markers to assess your body’s response to gluten, a protein found in wheat, barley, and rye.

Total IgA is checked first to rule out IgA deficiency - a condition that can yield false-negative results on other IgA-based tests.

Tissue Transglutaminase IgA (tTG-IgA), considered highly specific for coeliac disease, detects antibodies against the enzyme that modifies gluten peptides in the small intestine.

Gliadin IgA measures the immune response to gliadin (a gluten component) and can be helpful, particularly in young children.

Gliadin IgG can aid diagnosis in people with IgA deficiency or other immune complications.

GLUTEN AND COELIAC SCREEN BLOOD TEST

This is a complex investigation that checks the necessary biomarkers for coeliac disease and body's reaction to gluten.

Total IgA - Total IgA quantifies overall Immunoglobulin A production in the body. In the context of coeliac screening, measuring Total IgA helps identify IgA deficiency, a condition in which the body either produces insufficient amounts of IgA or none at all. If you have an IgA deficiency, IgA-based coeliac tests may give falsely negative results, so this biomarker serves as an essential first step in accurate diagnostic evaluation.

Tissue Transglutaminase IgA (tTG-IgA) - Tissue Transglutaminase IgA is the primary and most specific serological marker for diagnosing coeliac disease. When people with coeliac disease consume gluten, their immune system produces antibodies against tissue transglutaminase, an enzyme involved in the post-translational modification of gluten peptides. Elevated tTG-IgA levels strongly indicate an immune-mediated reaction consistent with coeliac pathology, making it a cornerstone of diagnostic testing.

Gliadin IgA - Gliadin is one of the main protein components of gluten. Gliadin IgA antibodies are another measure of the immune system’s reaction to ingested gluten. Though Gliadin IgA can be useful, particularly in evaluating younger children, it is generally considered less specific than tTG-IgA for coeliac disease. Nonetheless, it can provide additional clinical insights when combined with other biomarkers.

Gliadin IgG - Gliadin IgG testing is often used alongside or as an alternative to Gliadin IgA in people who have confirmed or suspected IgA deficiency. In such cases, IgA-based tests may not be accurate, so testing for IgG antibodies against gliadin can help detect an ongoing immune response to gluten. Elevated Gliadin IgG levels may warrant further diagnostic steps for coeliac disease or other gluten-related disorders.

    Yes, usually the same or next day.


    One Day Tests has a fantastic clinical team of consultants and GPs. Once you have your results you can book an online video consultation to discuss your results.


    Our clinical team consists of practicing NHS and private GPs, who work closely with our laboratory and can tell you what your blood test results mean for you. Having an integrated laboratory and GP services makes your health journey seamless and extremely efficient.

Coeliac and gluten investigations are not always straightforward.

You may need clinical interpretation, which takes into account not only the numeric values of the tests but also the patient’s medical history, symptoms, dietary habits, and any relevant comorbidities. Certain factors, such as selective IgA deficiency, recent dietary changes, or coinciding autoimmune disorders, can influence laboratory readings, necessitating professional judgement.

SPECIAL INSTRUCTIONS

NICE (medical regulatory and guidelines body) recommends including gluten in more than one meal a day for at least six weeks before testing.

Whilst we don't recommend any specific amount of gluten, some must be present for the testing to be accurate and the reaction levels measurable.

Why measure Total IgA?

Measuring total Immunoglobulin A (IgA) is critical in coeliac disease screening to rule out selective IgA deficiency, a condition wherein individuals produce little to no IgA.
