What is an Alanine AminoTransferase blood test?

An Alanine Aminotransferase (ALT) blood test measures the levels of an enzyme called ALT in the blood. ALT is primarily found in the liver, and its levels increase when the liver is damaged or inflamed. Therefore, an ALT blood test is often used as a marker of liver function and damage.

The test can help diagnose liver diseases such as hepatitis, cirrhosis, and fatty liver disease. ALT levels can also be elevated due to other factors such as alcohol consumption, obesity, and certain medications.

An ALT blood test is a simple and relatively inexpensive test that is recommended for individuals who have a history of liver disease or liver damage, those who consume alcohol excessively, and those who have risk factors for liver disease like obesity or diabetes.

Hi, I am Dr. Adam Staten and here's my view on this test as a GP and Clinical Director at One Day Tests.

Alanine Aminotransferase (ALT) is an enzyme produced by the liver. It is used as a marker of liver function/damage and high levels of ALT may indicate that the liver has been damaged.

This marker should be interpreted together with another liver enzyme, the Aspartate Aminotransferase (AST) which will give a more complete overall picture of the enzymes.

Reference ranges

If your indicative ALT level is lower than the reference range for our laboratory:

Having a lower-than-normal ALT result is uncommon and usually is not a cause for concern. Occasionally it could indicate a vitamin B6 deficiency or kidney disease.

Please discuss this result with your GP.

If your indicative ALT level is higher than the reference range for our laboratory:

Extremely high concentrations of ALT are often due to hepatitis and viral infection. The levels will eventually return to normal, though sometimes it may take up to 3 months. 

High levels of ALT concentrations are present in chronic hepatitis and vary between normal and elevated levels, so it is worth performing this test regularly to monitor results over time.

Alcohol and diabetes may lead to a moderately high level of ALT, causing increased fat levels in the liver.

You will need to discuss the results that are outside of the reference range with your GP.

Red flag symptoms for the Alanine Amino Transferase (ALT) Blood Test

Even if the blood test is normal, you should still see your doctor if you have any of the symptoms below. 

  • Jaundice (yellowing of the skin and eyes)
  • Abdominal pain or swelling
  • Unexplained fatigue or weakness
  • Loss of appetite
  • Nausea and vomiting
  • Dark urine
  • Pale stool

  1. SPECIAL INSTRUCTIONS FOR TAKING THE ALANINE AMINO TRANSFERASE (ALT) BLOOD TEST.

    Typically, an ALT blood test doesn't require any special preparations. However, it's common to order the test alongside other blood tests, which may require fasting. Each of our home test kits comes with instructions which explain what you need to do.


Venepuncture appointment - how to prepare

If you are coming into one of our clinics for the test, this is what you need to do before you arrive:

  • Drink plenty of water about an hour before the appointment.
  • Avoid coffee/tea/energy drinks. Water is good!
  • That’s it! We’ll do the rest.

Our blood test kits are simple and convenient.

The most important step in doing a blood test at home is to read the instructions very carefully. The process is very simple, but there are a few pitfalls you need to be aware of - you will save yourself a lot of time if you do it right the first time around!

  • Full printed instructions will arrive with your kit - please read them carefully.
  • Drink plenty before taking the sample and warm your hands up in the sink with warm water for a few minutes. This will make your blood flow much better.
  • Fill the tubes to the top line. We don’t need much blood; filling tubes to the top line is about 15 drops of blood.
  • Post the samples on the same day you take them, before midday, Monday to Thursday. If you post it any other time it might get to us late or be stuck at a sorting office over the weekend. You will then have to do it again.
  • Package the sample in the same box the kit came in. Otherwise, Royal Mail or DPD will reject it. The sample tubes go into a pouch, then into a cardboard box, and then into a return envelope. All of this is provided in the kit: please use it.
