What is HRT?

HRT is a medical treatment used to supplement or replace hormones that the body no longer produces in sufficient quantities. HRT can help alleviate symptoms caused by hormonal changes such as hot flushes, night sweats, mood swings etc, reduce the risks of the long-term conditions associated with the menopause such as decreased bone density and cardiovascular disease, and improve overall quality of life.

HRT involves the administration of hormones, primarily estrogen and progesterone, to restore hormonal balance. In some cases, testosterone may also be included.

There are two main types:

  1. Estrogen-Only HRT: Typically prescribed for women who have had a hysterectomy, as they no longer require progesterone.

  2. Combined HRT: A combination of estrogen and progesterone is used for women with an intact uterus to protect against the risk of endometrial cancer caused by estrogen alone.

HRT is available in various forms, including pills, patches, creams, gels, and vaginal rings, allowing you to choose the option that best suits your needs and preferences.

Hormone Replacement Therapy offers several health benefits, including:

  1. Symptom Relief: Reduces the severity of menopausal symptoms such as hot flashes and vaginal dryness.

  2. Bone Health: Protects against bone loss and reduces the risk of osteoporosis-related fractures.

  3. Heart Health: When started early in menopause, HRT may reduce the risk of cardiovascular disease.

  4. Improved Quality of Life: Alleviating symptoms can enhance overall well-being and mental health.

Risks and Considerations

While HRT is highly effective, it is not without risks. Potential risks depend on the type, dosage, and duration of therapy, as well as individual health factors.

These may include:

  • Increased Risk of Blood Clots & Stroke: Particularly with oral forms of HRT. Transdermal forms of HRT such as patches and gels etc is unlikely to increase the risk

  • Breast Cancer: Long-term use (more than 5 years) of combined HRT has been associated with a slightly increased risk.

Take the time to discuss any concerns you might have about Hormone Replacement Therapy and the risks with your doctor.

Who Can Use HRT?

HRT is suitable for many women, but it may not be appropriate for those with certain medical conditions, including:

  • A history of breast or endometrial cancer

  • Uncontrolled high blood pressure

  • Blood clotting disorders

  • Liver disease

Before starting Hormone Replacement Therapy, it is essential to consult a healthcare provider for a thorough evaluation, including a review of personal and family medical history.

Alternatives to HRT

Making the decision to use Hormone Replacement Therepy or to consider alternative methods is a very personal one. For some people they may not be able to take HRT or choose not to. There are other options you can consider:

  1. Lifestyle Changes: Regular exercise, a healthy diet, and stress management can help alleviate symptoms.

  2. Non-Hormonal Medications: Antidepressants, clonidine, and gabapentin may reduce hot flashes and mood swings.

  3. Complementary Therapies: Herbal remedies, acupuncture, and mindfulness techniques may offer relief for some women.

Further non-hormonal medications to help with menopause symptoms, are currently being researched, and will be available soon. Discuss your personal situation with your doctor to help make the right decision for you. The Women’s Health Concern has many helpful resource factsheets to help you in making treatment decisions.
