A subcutaneous injection is an injection into the fatty layer just below the surface of the skin.
Some people find it less painful than an intramuscular (IM) injection and is good when the volume of medication that needs to be injected is relatively small.
An SC injection is usually administered under the skin of the abdomen, but it can be given under the skin of the thigh or buttocks too. We generally recommend the abdomen as an easy site to reach.
SC injections are generally also easier to perform and don’t carry the risk of entering a blood vessel which occasionally occurs with IM injections.

An intramuscular injection is an injection into one of the big muscles of the body. Because muscles have a rich blood supply, an IM injection allows testosterone to get into your system quickly and effectively.
IM injections are usually delivered into the thigh, buttocks or upper arm although we generally recommend using the thigh as this is easiest to reach.

Is TRT safe?
Yes, when prescribed and monitored properly TRT is a safe treatment.
In fact there is an increasing evidence base showing the health benefits of TRT in men with TD.
To ensure that it is safe you will need regular blood tests and review appointments.
These are included in the price of your subscription.
Is it legal to buy TRT online?
Yes, but only via a prescription from an appropriately qualified medical professional.
Can I increase my testosterone levels without medication?
Yes, testosterone levels can often be increased by weight loss, increased exercise, reduced intake of alcohol and recreational drugs such as cannabis, or reduced use of prescription medication such as opioids.
Why do I need two blood tests to diagnose Testosterone deficiency (TD)?
UK guidelines advise that TD should be confirmed by two separate blood tests as hormone levels fluctuate throughout the day and from day to day.
The second blood test also screens for other hormonal issues, helps to assess for cardiovascular risk and ensures that it is safe for you to use TRT should you need it.
Is it better to use testosterone gel or inject testosterone?
This just depends on your personal preference. Both methods are effective at restoring testosterone levels.
Can I ever stop TRT?
Yes, but the symptoms of TD that you experienced before starting treatment may return.
How will my TRT be monitored?
We monitor your total testosterone level and full blood count every three months to ensure your dosage is correct.
Are there side effects of TRT?
As with any medication, there are potential side effects to TRT but these are minimised by safe prescribing practices and regular monitoring.
Mild side effects include fluid retention, acne, excess body hair growth, and increased urination.
Some men may experience gynaecomastia (development of breast tissue) and reduction in testicular size.
Men who wish to have children should not use TRT as it can reduce fertility.
TRT can also cause an increased number of red blood cells which can make the blood more ‘sticky’ which is a risk for blood clots and strokes.
It is for these reasons that TRT should be closely monitored.
Will TRT help me put on muscle?
The aim of TRT is to restore testosterone levels to the normal physiological range.
For men who have been suffering with TD this will usually improve motivation and will allow them to train more effectively which, in turn, will improve muscle bulk.
The aim of TRT is not the same as the use of ‘anabolic steroids’ when testosterone treatments are abused to achieve ‘supraphysiological’ (very high) levels of blood testosterone. This practice is unsafe.
Is TRT appropriate for professional athletes?
No, TRT is prohibited for professional athletes.
Are the medications self-administered?
Testosterone can be given in a number of different ways.
At One Day Tests we offer three options that can all be self-administered.
These include a gel applied to the skin, and two injectable options (Cypionate and Sustanon), one given subcutaneously and the other given intramuscularly.

What are the different types of TRT medication injections?
There are two types of injections available for your TRT programme. These are split per product.
- CYPIONATE testosterone injections use subcutaneous injections
- SUSTANON testosterone injections use intramuscular injections
When you sign up for a subscription you can choose. If you would like to change at a later date, or during your consultation - no problem, our pharmacists can move you onto a different option.
What are subcutaneous injections?
Subcutaneous injections go into fatty tissue under the skin for slow absorption. It is a shorter needle.
These are applied to the abdomen (belly fat, avoiding the navel area), or outer middle thigh.
It can also be applied to the outer upper arm.
What are intramuscular injections?
Intramuscular shots target deep muscle tissue for faster absorption (this is usually good for larger doses), requiring longer needles.
This is a more difficult injection to make, so requires some practice.
For TRT intramuscular injections, the best sites are the glutes (upper outer quadrant of the buttock) or the thigh (vastus lateralis muscle). This provides the easily accessible large muscle mass.

- Check the packaging of the needle and syringe. Do not use a needle or syringe if the packaging appears damage.
- Check there is no oil in the top of the ampoule. If there is then warm the ampoule in your hand and turn it back and forth until the oil runs into the main part of the ampoule.
- Snap the lid off the ampoule by taking a firm hold of the body of the ampoule with the dot on the neck of the ampoule facing towards you. Then snap the top off the ampoule by snapping it away from you. Dispose of the glass top in the sharps bin. NOTE: if you are using a vial then steps 5 and 6 do not apply but you should wipe the rubber top of the vial with an alcohol wipe and allow this to dry.
- Stand the ampoule on a flat surface. Take a green needle and syringe out of their packaging and attach the needle to the syringe. Remove the needle cover without touching the needle with your fingers, or allowing it to touch any other surface.
- Fill the syringe by putting the needle straight into the centre of the ampoule and gently pulling back on the plunger. If you are using a vial then put the needle through the rubber top of the vial, turn the vial upside down and slowly draw the plunger back.
- Turn the syringe upside down (needle pointing upwards) and check for air bubbles. If there are any air bubbles, tap the syringe so that the air rises to the end of the syringe and then gently push the plunger in to expel the air.
- Change the needle for a clean, blue needle.
- Identify your injection site.
- Clean the injection site with an alcohol wipe and allow it to dry,
- Gently pinch the skin so that you hold it away from the underlying muscle.

CLINICAL RELEVANCE AND CONFIRMATION
Testosterone deficiency (TD) is an increasingly common problem. Estimates vary as to its prevalence but some studies estimate up to 40% of older men are affected, with rates of testosterone deficiency tending to be higher in men who suffer from other medical problems such as obesity or Type 2 Diabetes. TD is often not recognised as men are sometimes reluctant to complain of the symptoms, and clinicians often do not consider it as a diagnosis.
